Auto-Adjudication Is a Provider Data Problem in Disguise

There’s a familiar moment in health plan operations: a claim enters the system, moves toward auto-adjudication and then stops. Something didn’t match. A manual reviewer picks it up. The queue grows. The cost accumulates.

Most organizations treat this as a claims problem. It isn’t. It’s a data problem and the data that’s failing them is provider data.

The Mechanical Link Between Provider Data and Adjudication Failure 

Auto-adjudication isn’t magic. It’s logic. A claim processes automatically when every field resolves cleanly against a set of rules: the NPI is valid and matches the system of record, the taxonomy code aligns with the provider’s credentialed specialty, the network participation status is current, the service location is correct. When any of those conditions fails (even one), the claim falls out.

Reworking a denied claim can cost between $25 and $181, adding significant overhead to already strained revenue cycle teams. Multiply that by the volume of claims falling out of auto-adjudication due to provider data drift, and you have a quantifiable, recurring operational cost with an identifiable root cause.

The specific denial codes tell the story clearly. CO-170 (provider not eligible), CO-8 (claim/service is not covered by the plan), and NPI mismatches are not random noise. They are the documented output of provider records that haven’t kept pace with reality; NPIs that exist in one system but not another, taxonomy codes entered at credentialing and never reconciled with what claims is using, network participation statuses that were accurate at the last roster update but have since drifted.

Claims that once cleared automatically now often run through complex rules, automated checks, and frequent manual rework, creating growing administrative costs and delayed revenue.

The Enforcement Signal: Payers Are Tightening Requirements

The pressure isn’t coming only from internal operations. It’s coming from the market.

UnitedHealthcare began enforcing NPI and taxonomy code requirements across its New York Medicaid network in 2024, with explicit requirements that both fields be present and valid on every claim submission. Optum Behavioral Health extended NPI and taxonomy enforcement to commercial ABA claims beginning in 2026. These aren’t isolated policy shifts, they reflect a systemic tightening of the data standards that every claim must meet.

What this means in practice: the tolerance for provider data drift is shrinking. Payers are building the enforcement into the adjudication engine itself. Organizations that haven’t resolved their provider data quality problems will see these requirements translate directly into higher denial rates and more manual intervention. 

The $21 Billion Case for Fixing the Input, Not Just the Output

The 2025 CAQH Index shows that U.S. healthcare has accelerated automation, interoperability, and AI adoption, yet a significant savings opportunity remains through further automation of manual and partially manual transactions. Industry analysis consistently points to a remaining $21 billion savings opportunity in administrative automation.  

The majority of those manual transactions don’t exist because automation is technically impossible. They exist because the data feeding the automation is unreliable. 

Auto-adjudication systems are sophisticated. The problem isn’t the adjudication engine,  it’s what you’re feeding it. Garbage in, manual queue out.

The Income Statement Consequence

Auto-adjudication rates aren’t an operational metric. They’re a financial one.

Every percentage point improvement in auto-adjudication directly reduces labor spend in claims operations, shrinks AR days, and improves cash flow predictability. Every percentage point of decline is a cost multiplier: more staff time, more rework cycles, more appeals, more days before a claim is paid or written off.

The ROI math is direct: a 0.1–0.3% lift in auto-adjudication rates typically offsets the full annual cost of a provider data management subscription. Most health plans are leaving multiple percentage points on the table, not because of claims system limitations, but because the provider data flowing into those systems hasn’t been continuously validated, reconciled, and trusted.

Consider the actual mechanism. A provider relocates to a new practice. The address in credentialing gets updated. The directory eventually reflects it. But claims is still routing against the old location,  because claims pulled from a roster file submitted three months ago. That single discrepancy generates denials. Those denials generate rework. That rework generates cost.

The fix was never in the claims system. It was in the data pipeline upstream.

Treating the Cause, Not the Symptom

The instinct in most organizations is to address auto-adjudication problems by tuning the adjudication engine: updating rules, adding exception handling, expanding the manual review team. These interventions manage the symptom without touching the cause.
